I have an Anderson rail type hitch. I pull with a f250 super duty 6.8 foot bed. My Anderson ball is all the way down, tripod hitch offset to the rear and the adapter coupler facing forward. This gives me a full 6 inches of clearance in all configurations.

I have a 2017 Ram 3500 Megacab with 6'4" box and I also have the Rambox storage boxes over the wheelwells and a tonneau cover. The length and width of my bed makes it more restrictive than most other combinations. I have the Andersen rail mount version. I do put the ball in the highest position and the coupler reversed (ball behind kingpin). I have 10" of clearance between the top of the bed rails and bottom of the camper. I can make some tight turns and still have clearance all around. I'm sure there is some scenario where I could damage something but I haven't come across that scenario yet. Here are some pics I just took of my clearances.
